Occupational Therapist - Community

What will your responsibilities be?
- Carry out functional assessments, reviews, and care planning
- Complete moving & handling assessments and provide interventions
- Recommend specialist equipment, seating, and home adaptations (minor & major)
- Promote independence through strengths-based, person-centred practice
- Work collaboratively with health, housing, and partner agencies

Qualifications & Experience:
To be successful in this role you must have,
- Community OT experience
- Strong knowledge of moving & handling, equipment, and adaptations
- Ability to work independently across a large geographical area
- Full UK driving licence and access to own vehicle
