Occupational Therapist – Reablement

What will your responsibilities be?
- Deliver reablement‑focused OT assessments and interventions
- Complete Care Act assessments, Mental Capacity Assessments, carers’ assessments and reablement reviews
- Undertake Moving & Handling risk assessments and create Personal Handling Plans
- Prescribe and recommend equipment and minor adaptations
- Maintain accurate case records on Mosaic (daily updates required)
- Work in line with Care Act 2014 and local authority statutory duties

Working Pattern:
- Standard 36 hours, Monday–Friday
- Optional overtime/weekend work available
- Hybrid role: council offices for visits, home‑based admin and case management
Qualifications & Experience:
To be successful in this role you must have,
- HCPC‑registered Occupational Therapist
- Strong knowledge of Adult Social Care legislation and OT practice
- Experience achieving positive outcomes through MDT working
- Excellent report writing and time management skills
- Reablement and/or local authority experience desirable
