Job Title: Social Worker - Palliative Care
Location: Calderdale
Pay Rate: £35 per hour
Job Description:
Liquid Personnel is recruiting Social Worker for its client's Palliative Care Team at Calderdale.
What will your responsibilities be?
- Holistic assessments and care planning: Work with individuals and families to assess emotional, social, and practical needs, ensuring care plans reflect dignity, comfort, and end-of-life wishes.
- Multi-disciplinary collaboration: Coordinate with healthcare professionals, hospices, and community services to deliver integrated support that addresses medical, psychological, and social aspects of palliative care.
- Advocacy and support for families: Provide guidance on benefits, housing, and care options, while offering emotional support to relatives during end-of-life care and bereavement.

Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
