Social Worker – Urgent and Emergency Care

Our client based in Nottinghamshire is seeking an experienced Social Worker to join their Urgent and Emergency Care (Hospitals) Team. This is an excellent opportunity for a motivated practitioner to work within a fast-paced hospital setting, supporting adults with complex needs to achieve safe and timely discharges whilst promoting independence, wellbeing, and positive outcomes.
The successful candidate will work within a multi-disciplinary environment alongside health professionals, community services, voluntary organisations, carers, and families. You will undertake strengths-based assessments, coordinate support services, manage risk, and develop person-centred plans that enable individuals to return home or transition to the most appropriate setting following hospital admission.
This role requires a confident and experienced Social Worker who can manage competing priorities, work effectively under pressure, and deliver high-quality social work interventions within urgent and emergency care pathways.
Key Responsibilities
- Managing a complex caseload of adults requiring social care intervention within hospital and urgent care settings.
- Undertaking Care Act assessments, reviews, and support planning to identify eligible needs and achieve positive outcomes.
- Completing health and social care assessments that support safe, effective, and timely discharge planning.
- Developing and implementing strengths-based, person-centred Care and Support Plans that maximise independence and wellbeing.
- Identifying opportunities for reablement, early intervention, and preventative support to reduce long-term dependency on services.
- Undertaking safeguarding enquiries, risk assessments, and risk management interventions in accordance with statutory guidance and local procedures.
- Supporting hospital discharge pathways and working collaboratively with health partners to remove barriers to discharge.
- Coordinating support services and community resources to enable individuals to return safely to their own homes wherever possible.
- Providing advice, guidance, and professional support to service users, carers, families, and partner agencies.
- Working closely with NHS professionals, community providers, voluntary organisations, and multidisciplinary teams to achieve integrated support and positive outcomes.
- Maintaining accurate, timely, and legally compliant assessments, reports, case records, and support plans.
- Contributing to practice development, mentoring, and the support of newly qualified staff, students, and less experienced practitioners where appropriate.
Qualifications & Experience
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Minimum of two years' post-qualification experience within Adult Social Care or a related setting.
- Hospital Experience: Previous experience within hospital social work, discharge planning, or urgent and emergency care services is highly desirable.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and relevant social care policy and guidance.
- Assessment Skills: Proven ability to complete complex assessments, reviews, support planning, and risk management interventions.
- Safeguarding Experience: Experience undertaking safeguarding enquiries and responding to safeguarding concerns effectively.
- Multi-Agency Working: Strong experience working collaboratively with health colleagues, community services, carers, and partner organisations.
- Communication Skills: Excellent verbal and written communication skills, including report-writing, assessment, and presentation skills.
- Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
- Organisational Skills: Ability to manage competing priorities, work independently, and perform effectively within a busy hospital environment.
